| Neurology
is a branch of medicine dealing with the diagnosis
and treatment of diseases and disorders of the
nervous system. The nervous system has two parts:
the central nervous system (including the brain
and the spinal cord) and the peripheral nervous
system (including the outside brain and the
spinal cord).
These systems are very complex in that they
can send and receive vast amounts of information
simultaneously. However, the system is also
fragile to disease and injury. Encephalitis
or meningitis can occur if bacteria or viruses
invade the brain or spinal cord, causing infection.
Nerves can degenerate (change from a higher
form to a lower form) causing Alzheimers or
Parkinsons. A blockage of blood supply to the
brain can cause a stroke. Also, injuries or
tumors can cause structural damage to the brain
or spinal cord.
Brain Surgery? Doesn’t it involve a threat
to life and limb? Not if the problem can be
handled through keyhole surgery.
|
|
| |
The
Keyhole Option
Modern endoscopes combined with image intensifiers have
made pituitary brain tumour surgery through a keyhole
incision, possible. |
| |
|
|
Pituitary
Problems
Pituitary brain tumours can develop at any age. The first
symptoms are visual decreasing vision and the inability
to see objects clearly from certain angles. It can also
produce endocrinological abnormality due to increased
secretion of normal hormones. Growth hormone excess in
children can lead to gigantism where the child may grow
to a height of more than 6 ½ feet. In adults it
leads to abnormal protrusion of the jaw, spade like hands
and feet and brittle bones (Acromegaly). Females may suffer
from excess of another pituitary hormone known as Prolactin.
This leads to cessation of menstrual flow, infertility
and milk secretion. |

| Stroke |
Stroke
is a disorder in which the arteries of the brain
become blocked or ruptured, resulting in death
of the brain tissue.
Interventional Neuro Radiology at Apollo is a
department dedicated to the prevention and recovery
of strokes. Using a machine that has biplane rotational
angio facilities, clots can be stopped from embolizing
(or travelling) and giant aneurysms can be treated.
Thrombolysis is the dissolution of a thrombus
or clot by injection of Urokinase directly into
thrombus, in the specific artery. Angioplasty
and stenting is also being done to prevent strokes. |
|
|
The Apollo Asha Biofeedback Center is well known for
its delivery of Biofeedback therapy for stroke and other
non progressive CNS (central nervous system) patients,
aiding them in recovering from their disabilities. This
technique involves the placing of surface electrodes
over the paralyzed or weak muscles and collecting the
dull signals from the brain and spine. These signals
are then amplified and shown on computer to the patient
so that they can learn what to do to make signals stronger.
Through trial and error learning, the patient is able
to make new pathways in the brain and spinal cord creating
permanent positive changes in their functioning ability.
These changes are reinforced with exercises and activities.
*CP (Cerebral Palsy), SCI (Spinal Cord Injury) and other
head trauma also respond to this therapy.

Brain
and Spinal Tumors
Tumors of the brain and spine may be malignant (cancerous)
or anti neoplastic (non cancerous). No one really knows
the origin of these tumors but it is known that brain
tumors are more common and seem to have equal occurrence
in men and women. Quality of life is diminished through
neurological deficits with these tumors and they must
be dealt with as soon as a definitive diagnosis is made.
The dedicated neurosurgical
team at the Institute of Neurosciences at the Apollo
hospital aim not only to preserve life, but to eliminate
neurological deficits while decreasing pain and discomfort
to the patient. About 1000 neuro surgeries are performed
every year using the most advanced equipment available
including: |
| |
|
|
-
The Clinas 6000SR Linear Accebrator
with X-Knife is a highly sophisticated, computer
driven technology used for its precision in removing
appropriately selected tumors of the brain and spine,
arteriovenous malformations, and other abnormalities.
-
Frame based Stereotaxic surgery
generated RF lesions (radio frequency energy) has
been very successful in neurosurgical procedures
like pallidotomy (stereotactic surgery in which
lesions are produced in the globus pallidus for
treatment of extra pyramidal diseases) and in thalamotomy
(stereo tactic technique for the discrete destruction
of specific groups of cells within the thalamus,
done to relieve pain, to alleviate the tremor and
rigidity in paralysis and in psychosurgery to relieve
certain anxiety states , psychoses and obsessive-compulsive
disorders).
-
Microsurgical Instrumentation
using the least invasive technique to accurately
excise tumors of the brain and spine has been particularly
successful in lumbar discectomy (disc removal from
the lower spine).
|
| |
|
|
| Degenerative
Disc Disease |
The
inner portion of the disc is soft and spongy and enables
it to function as a shock absorber. Over time the intervertebral
discs degenerate, losing their water content and ability
to absorb shock with movement. Excessive pressure, strain
or injury to a rigid disc can cause the disc to tear
and bulge. This causes the discs to come closer together
and compress the spinal nerve roots, causing pain, numbness
and decreased mobility.
At our partner hospitals the highly advanced skills
of the neurosurgeons include Computer Assisted Neurosurgery
using an operating computer, which allows them to apply
instrumentation within 1 mm of accuracy. Automated percutaneous
lumbar discectomy is done this way. The device they
use is also very advantageous in locating difficult
tumors and avoiding fragile structures during surgery.
Microsurgery (dissection of minute structures under
microscope by hand held instruments) can be successfully
done for lumbar or anterior cervical discectomy or decompression.
Total disc replacement surgery is also available when
the degeneration of the disc or discs is so far advanced. |

| Epilepsy |
Epilepsy
is a disorder in which abnormal electrical impulses
discharge abnormally, resulting in temporary brain
dysfunction or seizure. Some seizures can be life
threatening and very frightening as the length
and duration of seizures is unpredictable.
Wockhardt Brain and Surgery hospital have a
Specialty Epilepsy clinic which uses Computer
assisted neurosurgery to manipulate the discharging
neurons to help stop seizures and give the patient
back control of their life.
